Course Description

Tier 1 behavioral intervention in the school setting is a much-needed and under-served area of OT practice. This course will introduce participants to 1-2-3 RESET™, a trauma-sensitive framework that incorporates evidence-based approaches to support students’ ability to regulate their bodies and emotions. While SEL curriculums focus on topics like talking about feelings and teaching social skills from a cognitive perspective, they minimally address the foundation of these skills- body-based regulation. Recognizing a vital need in the educational landscape, this framework helps bridge the gap between therapy and the classroom by incorporating whole-class Tier 1 intervention strategies focused on the five principles  of 1-2-3 RESET™: Relationship, Environment, Self-regulation, Empowerment, and Task. 

During the course, lecture-based instruction, experiential activities and small group discussions will equip participants with an in-depth understanding of trauma and its impact on learning along with practical, easy-to-implement strategies teachers can use in their classrooms to help students thrive. Participants will leave the course armed with an 8-week curriculum to help children regulate their bodies and emotions, 38 useful handouts, and a plan for implementing 1-2-3 RESET™ concepts into classrooms the very next day. The 1-2-3 RESET™ framework can be used by a variety of professionals to help children learn how their brains work, practice identifying sensations in their bodies, explore how those sensations connect to emotions, and discover strategies to help themselves regulate. While this course focuses on school-based interventions, 1-2-3 RESET™ is easily adaptable for individual therapy/clinic use as well.

1-2-3 RESET™ is a new framework that is based on extensive evidence in the areas of occupational therapy, sensory integration, education, neurobiology, neuroanatomy, trauma, and psychology. Although there is not published research on the approach yet (a future goal!), preliminary data collected from teachers using 1-2-3 RESET™ in their classrooms shows the following:
-Increased teacher knowledge of the impact of trauma on learning
-Increased use of embedded strategies in the classroom by teachers and students
-Increased student knowledge of their brain and strategies to support learning
-Increased student regulation, which in turn resulted in improved ability to participate & learn

While oral-motor treatment approaches for neurologically impaired children have focused on severe feeding dysfunction, recent clinical work with children who have sensory processing/developmental dysfunction has revealed a strong association between subtle oral-motor and respiratory dysfunction and many sensory integrative and sensory motor problems. 

 

A framework for understanding how and why many aspects of development are significantly influenced by oral functions will be presented. This discussion will include the relationship of the suck/swallow/breathe synchrony to behavior, learning, postural development, feeding, communication, arousal, self-regulation, psychosocial development, and even common health issues in children. The relevance of this information to treatment principles with children who have sensory processing/developmental dysfunction will then be shared using slides, case examples and participant lab activities. Documentation strategies for identifying abnormal function and demonstrating change through treatment will be provided. This information is particularly useful for occupational and physical therapists and speech pathologists working with children but may be applied across the age span.

This workshop provides clinicians with a grounding in Polyvagal Theory and revisits key concepts of OT-SI intervention in order to explore key points of alignment and distinction between these two frameworks. ADHD (Attention Deficit Hyperactivity Disorder) is a common diagnosis in pediatric practice today. Attention deficit can be a diagnosis, but it can also be a symptom of underlying neural inefficiency. As we dig deeper into sensory processing, we discover multiple facets that forms part of a developmental hierarchy that enables us to use clinical reasoning in both assessment and intervention. Dr. Russell Barkley and Dr. Edward Hallowell stress the importance of self-regulation and self-reflection as key components to consider, both of which overlaps into sensory processing practice. A sensory diet is an occupational therapy intervention that offers an opportunity to support sensory modulation-based needs that impact general regulation on a daily basis.  The Sensory Diet is a term that has become commonly used in pediatric occupational therapy, though the intention and integrity of the concept is often poorly understood.

As originally described by Patricia Wilbarger, in the early 1970's, the sensory diet was designated for the treatment of sensory defensiveness.  With advances in understanding of sensory modulation difficulties, the sensory diet concept has emerged as a powerful strategy to address not just sensory defensiveness, but other sensory modulation difficulties including arousal regulation, emotional regulation, attention systems disruption, as well as to support socially mediated engagement.  When provided correctly, the sensory diet approach offers a comprehensive method that includes: 1) education and awareness, 2) an individualized, targeted sensory diet plan that is professionally guided and monitored, and 3) specialized, professional treatment techniques targeted at key issues of concern. 

 

In this course, you will learn how to establish power, precision and proper timing in the sensory diet targeted to desired outcomes. The evidence-base supporting the use of a sensory diet approach will be reviewed. Hands-on practice will encourage participants to understand the nuance of application.

 

Implementation of a sensory diet involves parents and other adults, so coaching them to effectively implement the sensory diet is included in the course, as is monitoring the effectiveness of the program. Case examples from children with neurodevelopmental needs will illustrate the power of matching the sensory diet to a child’s specific needs.  Finally, the sensory diet concept will be contrasted with a sensory based approach, including the "sensory lifestyle”, sensory breaks, and sensory supports, accommodations, & modifications.

Gravity influences infant development in many ways, from triggering vestibular processing to developing and strengthening muscles that enable humans to gain and maintain upright posture. This course will evaluate a number of factors impacting antigravity development that affect performance in sensory processing and modulation, sensory motor functioning and psychosocial behavior throughout the age span. Obstacles to typical development and indicators of delays and/or atypical function in the particular areas of core strength, proximal stability and motor coordination essential for life skills will be discussed.

 

Along with other treatment strategies, gravity can be used by therapists to enhance development, especially in the areas of sensory motor processing, antigravity motor control and motor coordination. A variety of intervention strategies that engage vestibular mechanisms and take advantage of gravitational influences, including some innovative equipment will be demonstrated via participant labs and video examples.

Equipping as a Healing Practitioner: Irene Ingram
Practitioners evolve with repertoires of skills that lead them to their individual gifting. Setting goals for oneself such as “I want to be able to engage in amazing ways”, is the necessary catalyst for acquisition of greater gifting in an area. Understanding the relationship of self with the spirit of healing, the power of imagination and vision, and limiting or empowering beliefs will be explored during this portion of the course.


Relationship as a Healing Force: Kim Barthel
Present and connected relationships are a powerful healing force. They are an intrinsic modulator, a co-regulator of arousal states, and a motivator for all kinds of positive change. With secure relationship at the core of any intervention or frame of reference, there is much reason for hope. It is my belief that therapeutic relationship skills can be cultivated; they can be taught, and they can be learned. What is attunement? How do we learn to connect to ourselves so we can better connect with others? How do we harness the advantages of understanding non-verbal communication? These are some of the questions that will be explored during this portion of the course are intended to deepen our compassionate self-inquiry – towards supporting our relationships whether they are in clinical settings or far beyond.

 

Developmental Expression of Engagement: Patricia Oetter
This last section of our conference will focus on the development of connection and engagement from womb to school age. It will be done in the context of my Extended Matrix Model which was somewhat based on the work of Joseph Chilton Pearce. His work considered social emotional development in the developmental stages of Womb, Mother, Kid Power and Brain Power. The Extended Matrix Model Considers these stages from the perspectives of Task, Environment, and Interaction. It will also integrate
the interdependent concepts of co-regulation and regulatory functions with the development of engagement and connection skills.
